Scalpel is a cutting-tool treatment item in Animal Hospital (Anomaly) used for Surgery cases, and one of five treatments — alongside Antibiotics, Scissors, Transplant, and Organ — that are exclusive to Room 8 and never called for in the five standard Medical Wing rooms. It also shows up outside the treatment loop entirely: it's one of the items the recurring character Barney asks for during his multi-visit storyline at the check-in window, coming after he's already asked for coffee, to protect his briefcase, and to take his photo.

Tips & Strategy

  • Keep it stocked for Room 8 surgery cases where it's called for.
  • Remember it's also one of Barney's specific requests during his storyline — giving it to him keeps that questline progressing toward his final visit.
  • Not sold in the Supplies Shop; obtained via diagnosis.
  • Barney can appear up to three times in a single shift between Shift 2 and Shift 7, and each request — including Scalpel — has to be handled within 120 seconds or he leaves; closing the shutters refuses him with no penalty besides missing that visit's reward.
  • Neither shooting nor tasing has any effect on Barney, unlike most other threats, so don't waste ammo or a Taser charge trying to deal with him that way.
  • Scalpel is one of five Room 8-exclusive treatments, so if a normal Medical Wing diagnosis seems to call for it, double-check you're reading the right room's request.
