Shift 1: The Tutorial Shift
The first of the Animal Hospital Roblox shifts is fully scripted and contains no anomalies. Two patients arrive: the Room 1 patient needs Eyedrops for Dried Eyes, and the Room 2 patient needs Herbs for Stomach Ache. Treat both correctly and Dr. Harlow unlocks the shutters and camera system for every shift after this one.
Shift 2: Story Characters and the First Anomalies
Regular patients continue, but this is also where the game's story characters can first appear — Barney, Ron from Accounting, and Ratthew have all been documented showing up from Shift 2 onward — alongside the first enemies and anomalies. Finishing Shift 2 unlocks the Emergency Wing (Rooms 6-8) and the Supplies Shop, run by the Shopkeeper Nurse NPC.

Shift 3: The Taser Unlocks
By the end of this shift, a free Taser becomes available and Dr. Harlow reminds players to start checking security cameras and polaroids for anomaly clues. This is the last of the early Animal Hospital Roblox shifts before the pace picks up.
Shift 4: The First Ambulance Event
The end of Shift 4 triggers a panicked broadcast from Dr. Harlow, featuring the News Bunny and warnings about Shapeshifters — followed immediately by the first Ambulance Event. Six patients rush in at once, some possibly anomalies, each with an individual death timer; completing the event awards a shift-end pay bonus. Shift 6 and Beyond: The Failure Condition
Shift 6 is Dr. Harlow's last scripted appearance in these Animal Hospital Roblox shifts — he warns that 3 animal deaths in a single run count as a failure condition. After Shift 6, no new scripted milestones are documented; the loop simply repeats, with the Ambulance Event returning every 5 shifts.
