Barney

A recurring 'Returning Character' and presumed major antagonist, first appearing between Shift 2-7 and able to appear up to three times in one shift.

Barney is one of the most story-heavy characters in Animal Hospital (Anomaly), a recurring 'Returning Character' who first shows up between Shift 2 and Shift 7 and can visit the check-in window up to three times in a single shift. Across his visits he escalates from asking for coffee, to asking you to hide his red briefcase, to requesting his photo be taken — the moment his anomaly form is first revealed — then a scalpel, and finally to be hidden from the police. He is later revealed to be a serial killer connected to the attack on Officer Duckman.

Behavior

A recurring 'Returning Character' and presumed major antagonist, first appearing between Shift 2-7 and able to appear up to three times in one shift. On repeated visits he asks for coffee, then to keep his red briefcase safe, then to take his photo (first reveal of his anomaly form), then for a scalpel, then to hide him from the police. If refused (shutters closed or no coffee given within 120 seconds) he verbally threatens the player but never follows through, and simply stops returning. He is revealed to be a serial killer who used a kitchen knife to incapacitate Officer Duckman.

How to Respond

Give him the requested item (usually coffee) within 120 seconds, or close the shutters to refuse with no other penalty besides losing the associated reward. On his final visit he asks to hide behind the front desk; choosing not to turn him in to Officer Duckman rewards a second coffee machine with a 300-second cooldown. Shooting or tasing him has no effect, unlike other anomalies.

Quick tips

  • Give him whatever he asks for (usually coffee) within the 120-second window, or close the shutters to refuse — refusing only costs you the visit's reward, nothing else.
  • He never follows through on his verbal threats if refused; he simply stops returning.
  • Shooting or tasing Barney does nothing, unlike every other anomaly on this list — don't waste ammo or a Taser charge on him.
  • On his final visit he asks to hide behind the front desk; declining to turn him in to Officer Duckman unlocks a second coffee machine with a 300-second cooldown.

FAQ

Can you fight or taser Barney?
No. Barney is explicitly documented as unaffected by shooting or tasing, which sets him apart from other Animal Hospital anomalies that can be incapacitated that way.
What happens if I refuse Barney's requests?
He verbally threatens you but never acts on it, and simply stops returning — refusing only costs you that visit's reward.
Is Barney the main villain of Animal Hospital?
The wiki documents him as a presumed major antagonist and revealed serial killer tied to the attack on Officer Duckman, though his full role is still part of the game's ongoing lore.
